Primary or secondary root canal treatments aim to eliminate the microbial, seal the root canal space and resolve the peri-apical lesion; however the anatomical complexity as well as some extra-radicular infections make it necessary that an endodontic surgical approach is called for in some cases. All the dental treatments are risky, but patients and dentists are usually really afraid about endodontic surgery. What are the reason of this fear? Is the surgical approach a suitable option for saving natural teeth? What are the risks related to this kind of treatment and it is more risky than a non surgical approach? This presentation will try to answer to these questions showing a detailed revision of letterature and sharing my experience on this issue.
