The presence of bacteria on the implant surface may lead to inflammation of the mucous membrane around the implant. If this inflammation is not treated, it will spread in the apical direction, which will lead to resorption of bone tissue and possible loss of the implant. For the treatment of periimplants, it is important to remove bacterial plaque and inflammatory tissues, to protect the surface of the implant. Er: YAG laser is able to remove infected soft and hard tissue around the surfaces of implants. Since this wavelength is poorly absorbed by the titanium surface of the implant, the temperature around the implant during irradiation does not significantly increase, therefore, does not lead to pyrolytic effects.

The purpose of this clinical trial was to evaluate the effectiveness of the use of Er: YAG laser in the treatment of peri-implants in combination with photodynamic therapy.

Materials and methods: Under supervision were 25 patients with 42 implants. All our patients have been diagnosed with peri-implants. The patients weredivided into two groups. First group: 10 patients (15 implants) were treated with Er: YAG laser (160 mJ, 10 Hz). Second group: 15 patients (27 implants) were treated with Er: YAG laser in combination with photodynamic therapy (7.3 W, 690-905, 60, in combination with toluidine blue). PDT was used immediately after laser purification and removal of granulation tissue. After that, bone plastics were made on the surface of the implant. Depth pockets, level of gum attachment and X-ray examination data were evaluated before treatment and once a month for six months.

Results and discussion: In both groups, the treatment results were positive: decreased pocket depth, gum attachment and no bleeding during the sensation, bone structure restoration. In the first group, in 80% of patients, a complete positive result was evaluated in 5-6 months.
